Care Collective exists to connect people with the resources, organizations, and support systems they need to move forward. We create the pathway by identifying needs, connecting individuals with trusted community partners, and helping remove barriers that stand in the way of progress.
But our work does not end with a referral.
We walk alongside people through encouragement, mentorship, accountability, and follow-up. We help individuals stay connected, keep appointments, overcome obstacles, and continue moving toward lasting stability and victory.
